1. The feature film, MFTIO will be shown on the dates indicated above. The host organization will not have private screenings nor will the host organization or any of their guests make any copies of the DVD.

2. Enlightened Professionals will mail out two copies of the DVD two weeks prior to the premiere weekend. It is the responsibility of the host site to play the DVD prior to the event to ensure that the copy is not damaged. If there is a problem, a new copy will be sent to the host site. Instructions for submitting payments and how to mail back the DVDs will be included in this package.

3. The recommended price per attendee per ticket is $10.00 USD. (Canadian hosts charge a minimum of $12 CAD).

4. The host organization agrees to pay a minimum of $250.00 USD for 50 attendees, or less than 50 attendees for their screenings of MFTIO. The $250.00 USD is the minimum total for all screenings, not per screening. The host site also agrees to pay $5.00 USD for each ticket sold or otherwise issued beyond 50 attendees.

5. The host organization will be responsible for all costs associated with the screenings under their contract (equipment, comp tickets to volunteers, advertising, tax, etc.).

6. Within 48 hours of your screening dates, contact Enlightened Professionals at 860-232-3331 or musicmovie@epllc.org with the number of attendees at your events. Payment, Post Event Information Form and any outstanding copies of the DVD for the MFTIO screenings are due back two weeks from the date of your last contracted public screening. The organization will obtain a tracking number from the courier or postal service to ensure delivery of payment and the DVDs.

7. Payment will be received after two weeks of the screening date by one of the following payment methods: Check, Money Order or Credit Card: § Please send and make Checks or Money Orders payable to: Enlightened Professionals , 41 Crossroads Plaza – Box 144, West Hartford, Ct. 06117 -USA § Credit card payment will be accepted in the following manner by Telephone: 860-232-3331. A late fee of $25.00 USD will be charged if payment is not received by this date.

9. For cancellations any time prior to the week before the event, a $75 cancellation fee will be charged to the Host Site for removal of their information from www.emergingpictures.com. Cancellations within a week of the showing will be charged the $250.00 minimum.
